You may start, stop, increase, decrease, or change the tax designation of your personal contribution to the UMPIP at any time. Complete the UMPIP Contribution Agreement form. Give a copy to your treasurer and send the original by mail or email to benefitsteam@nccumc.org. The effective date of enrollment or changes must be the first of the month following the month in which you submit the form.
