IRS Form 1095 for the 2025 tax year is available upon request for conference health plan enrollees. While this form is not required to file your personal income taxes, it may be used to verify coverage. If you wish to receive a copy of your Form 1095, you may request it by:

• Calling Wespath Customer Service at 1-800-851-2201 business days from 8:00 a.m. to 6:00 p.m, Central time, OR
• Sending an email to ActiveTeam@wespath.org, OR
• Mailing your request to Wespath, Attn: Customer Service, 1901 Chestnut Ave., Glenview, IL 60025

Forms will be provided within 30 days of the request.
