About the Recommender

This recommendation form is for district superintendents and serves as the BOD required letter of recommendation for laity who will be certified or re-certified as a CLM. This form can be filled out before or after the CLM/candidate meets with dCOM and is needed every two years for CLMs to maintain active certification.

Assignment & Clergy Mentor(Required)

Once certified, it is the district superintendent’s role to assign the CLM to a ministry where they will serve as a part of a ministry team. This assignment includes naming a clergy mentor to oversee their ministry.

This CLM will be assigned to work within their local church.

This CLM will be assigned as a lay pastor of another congregation.

Clergy Mentor(Required)

All CLMs work as part of a ministry team and need a clergy mentor. When a CLM is assigned to a ministry in their local church, their pastor is usually the clergy mentor. When CLMs serve as a lay pastor, the DS assigns another clergyperson to serve as the clergy mentor. If you have not yet selected a clergy mentor, please simply write “to be determined.”
