The NC Conference Committee on Lay Servant Ministries requires background checks for all persons seeking certification or recertification as a certified lay speaker or certified lay minister (CLM). This requirement reflects our commitment to the safety and protection of children, youth, and vulnerable adults.
Background checks are conducted confidentially through Trak-1, a company the conference uses for background screening services.
Completing the Background Check Process
To complete the process:
- Download and complete the screening permission form.
- Submit the completed form according to the instructions provided on the form at least one week before your district committee interview.
- Submit payment by check or through the online payment portal. Fees are listed below:
- Simple Background Check (for certified lay speakers and CLM renewals): $10
- Background Check with Driving Record (for certified lay minister candidates): $25
Questions about Providing Your Social Security Number?
We understand that some applicants may be uncomfortable including a Social Security Number on a form sent by email or mail.
