ELDERS:
Extension Ministry Report (This report is available July 1 – 31, 2025.)
There are three sections in this form –
Part 1 – Appointment to an Extension Ministry,
Part 2 – Questionnaire, and
Part 3 – Committee Report.
Once you hit Submit, you should receive your copy via email. Your report will be automatically sent to the Bishop’s Office, Office of Clergy Life, District Office, and United Methodist Endorsing Agency, if applicable.
Only a copy of Part 1 – Appointment to an Extension Ministry needs to be sent to:
- Bishop of the area in which you serve, if other than the area of which you are a member
- A copy of this may be used to inform the Charge Conference(s) of which you are a member and an affiliate member in keeping with Paragraphs 317.1 and 344.3 a,b.
DEACONS:
All new provisional or full deacons appointed to extension ministry must submit the extension ministry report as well as the Deacon Annual Report required by the GBHEM (see below).
Extension Ministry Deacon Report (This report is available July 1 – 31, 2025.)
Only a copy of the Deacon Annual Report needs to be sent to:
- Bishop of the area in which you serve, if other than the area of which you are a member
- Charge Conference
