January 1, 2012 – December 31, 2012. Deadline: January 31, 2013

There are three sections in this form - Appointment to an Extension Ministry – Questionnaire – Committee. Please check box to receive a copy of your responses. Once you hit Submit, you should receive your copy via email and also your reports will be automatically sent to the Bishop’s Office, Ministerial Relations Office and District Office.
Only a copy of the Appointment to an Extension Ministry part needs to be sent to:
- Bishop of area in which you serve, if other than area of which you are a member.
- A copy may be used to inform the Charge Conference(s) of which you are a member and an affiliate member in keeping with Paragraph 316.1 and 344.3 a,b.
- A copy of this should be used to inform the United Methodist Endorsing Agency, PO Box 340007, Nashville, TN 37203-0007 in keeping with Paragraph 344.1b.
