The NC Conference Health Ministry Committee seeks applications from a local congregation interested in improving healthcare outcomes with persons in their community who may be uninsured or underinsured. In particular, this grant would be awarded to a church with a licensed healthcare provider willing to monitor outcomes for hypertension and diabetes care, including funding for first-line pharmaceutical treatments. Grant funds would be used to purchase monitoring supplies such as glycosated hemoglobin (HgBA1C) kits and blood pressure cuffs.
This grant would be approximately $5,000 and awarded in the last quarter of 2026 for initiation of work in 2027. Deadline for applications is October 31, 2026.
Recipients of the grant will be required to submit a report by the end of the year detailing their project results and receipts/invoices for grant money spent.
