This screening is open to enrollees (<strong>participants and spouses<\/strong>) on the conference\u2019s active health insurance plans.<\/p>\n\n\n\n<p>Attendees will receive:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>$100 in Reward Cash in their Personify Health well-being program. This incentive reward is redeemable as a transfer to your checking account or can be redeemed for gift cards, merchandise, or a charitable donation.<\/li>\n\n\n\n<li>Up to 140 Wellness Credits for achieving\u00a0healthy results. Earn a total of 150 Wellness Credits before the end of the year and receive an additional $150 in Reward Cash.\u00a0\u00a0<\/li>\n\n\n\n<li>Free, personalized, confidential data on 30 health factors that can be shared by the participant with their primary care provider. Healthcare providers have indicated that this valuable information supports them in creating an overall plan for the health of their patients.<\/li>\n<\/ol>\n\n\n\n<p><strong>Schedule Your Blueprint for Wellness Health Screening<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Log in to your\u00a0<a href=\"https:\/\/login.personifyhealth.com\/auth\/realms\/platform\/protocol\/openid-connect\/auth?client_id=platform-ui&amp;redirect_uri=https%3A%2F%2Fapp.personifyhealth.com%2F&amp;state=dd70b950-1114-42aa-8c8b-073624e06116&amp;response_mode=fragment&amp;response_type=code&amp;scope=openid&amp;nonce=d3960a57-d56e-4630-a691-6e438b1c9ffe\">Personify Health<\/a> account\u00a0and find Quest Blueprint for Wellness Screening under the \u201cBenefits\u201d tab or through\u00a0<a href=\"https:\/\/my.benefitsaccess.org\/login\" target=\"_blank\" rel=\"noreferrer noopener\">Benefits Access<\/a>\u00a0(for participants) under the \u201cWell-Being\u201d tab.<\/li>\n\n\n\n<li>Call Quest Diagnostics at 1-855-623-9355.\u00a0 Employer group = Wespath2026<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">Health Check<\/h2>\n\n\n\n<p>This year\u2019s results can be entered in the Personify Health Check.
