Has the church taken the required steps to satisfy all NCC Disaffiliation Agreement terms and conditions, including but not limited to the following items (for additional details, please see Step 5 of the NCC Disaffiliation Process):

5.a.i-v. Remit required payments using the NCC Disaffiliation Payments Remittance Advice form.
5.a.vi. Submit documentation of repayment of UM-/NCC-Affiliate obligations (if applicable).
5.b. Submit documentation of satisfaction, assignment, or transfer of other liabilities (if applicable)
5.c. Cease use of United Methodist intellectual property as of disaffiliation date.
5.d. Cease use of Group Tax Exemption as of disaffiliation date (if applicable)
5.f. Provide Certificate(s) of Insurance for liability coverage through disaffiliation date and reporting endorsement (if applicable).
5.g. Provide remnant congregation information to NCC/District Superintendent (if applicable).
5.h.i. Submit church records and archives to NCC Archives and History.
6. Prepare for organizational transition as of disaffiliation date.
